Due to growing demand for care services in and around Merton and surrounding areas and our local reputation for providing high-quality well-trained carers, we are looking for a compassionate Senior Home Care Assistant to join our team.

Being a Senior Daily Home Care Assistant means that you will need a full driving license and access to a car (covered by valid business insurance). We warmly welcome empathetic and compassionate applicants with a proactive attitude to life.

Role Purpose

 1.    To support adults with companionship and/or personal care needs to enable them to live within the comfort of their own homes.  To provide care in accordance with current best practices in line with Trinity policies and procedures, legislative requirements, and CQC regulations.

2.    To supervise the provision of care services and assist the Care Manager and Care Coordinator by providing field support to clients, existing Care Workers and new members of staff in order to maintain the highest standards of care.

3.    To supervise and support new and existing care workers in the field with on-going needs.

 Accountabilities

Assist with induction of new staff and development of existing staff

  • To assist with the shadowing and assessment of new Care Workers following Trinity Induction Training to ensure they are deemed competent to commence working alone in the field.
  • Offer support and help with corrective practice until they are confident to work alone.
  • To act as an ambassador for Trinity/buddy for new and existing care workers by leading by example and setting high levels of care practice at all times.
  • Ensure best practice is adhered to in client’s homes and in the field.
  • To assist individual staff members with personal development in their roles and highlight areas for development/training requirements.
  • To establish professional employer/employee relationship ensuring personal boundaries are not crossed and company policies and procedures are adhered to.
  • To attend update and/or self-development training when required in order to maintain own professional development and standards.
  • Seek opportunities for personal and professional growth.

 

Support Care Managers, Care Coordinators and Registered Manager from the field as required

  • Monitor and ensure quality of care for clients by conducting client reviews in a timely fashion to obtain feedback on level of service, quality of care workers and client satisfaction.
  • Plan work to complete spot checks and reviews within agreed time scales to include medication risk assessment, (Medication and Recording practices are followed in line with policy and procedure) and real time update of daily care activity through our digital platform [Mobizio ].
  • Recording changes to care plans as  necessary through Mobizio  and supporting any input within People Planner (our in-house rostering system )
  • Monitor and record the general health of clients and any change in circumstances and report this to the Care Coordinator and Care Manager keeping strong lines of communication.
  • Ensure personal support plans and risk assessments including medication risk assessments are being adhered to.
  • Complete spot checks on Care workers in the field to ensure high standards of care are being delivered
  • Support the implementation and development of Trinity’s policies and procedures and maintenance of standards required by relevant legislation
  • Escalate and report issues to the Care Manager/Registered Manager as appropriate.
  • Assist in the office if the need arises to ensure the office is covered with necessary numbers of staff
  • Responsibility for submitting expenses and maintaining bookings in line with our data quality procedures and practice.
  • Dress code – to wear Trinity uniform at all times.

Emergency Care

  • Respond to clients’ illness by calling GP/ ambulance and report back to the office.
  • Systematically solve day-to-day problematical issues which arise.
  • Pick up care visits in emergency situations if the office are unable to cover the calls (within stipulated availability)

 

On Call

  • To participate in the on call rota, acting as a point of contact for client or Care Worker issues whilst on call.
  • To resolve rota and other issues whilst on call to ensure the care calls are covered and the quality of care is maintained.
  • Record and report all actions undertaken whilst on call on Mobizio and inform the office as required .
  • Ensure calls are covered in the mornings before office opening and give full handover to coordinator in the morning to report/escalate any complaints or incidents and ensure continuity of care

 

Other

  • To develop effective working relationships with other Senior Care Workers, the Care Manager, Care Coordinator and other members of the office team
  • To promote Trinity as a provider of choice and also an employer of choice in line with the Company growth plans and recruitment targets.
  • Support an open, positive and inclusive working culture
  • support the effective resolution of team conflicts
  • To assist the Care Manager and Directors in other aspects of the business as requested
  • To undertake any other responsibilities and duties as appropriate to the nature and salary grade for the post

           

Following Procedures and Guidance

  • Maintain awareness, understanding and adherence to Trinity policies and procedures, current legislation and CQC requirements
  • Attending training sessions as required Participating in corporate and statutory initiatives as required
